Brad Clark Hypnosis Performance Contract

2. Performance Fees/Expenses $                           Paid in full immediately following performance of show.  Payment Instructions:  Please remit a deposit of 30% (                 ), made payable to Brad Clark, along with a signed copy of this contract within 10 days of receipt to guarantee performance date, time, and fees. 
The balance of payment 70% (                       ), is due and payable immediately following the performance.  (    )One nights lodging or $89 to be included.

3.  Performance Cancellation by the above Purchaser:  Once signed, this is a binding contract to all parties listed above.  If the purchaser cancels the performance, for any reason, the full performance fees are due and payable immediately at time of cancellation.  If the show is cancelled by the purchaser due to weather conditions, the original performance can be rescheduled up to 1 year from date of cancellation.  (Additional fees may apply and rescheduled show dates may or may not be available by Brad Clark depending on other show contracts already made at cancellation time.) 

4. Performance Cancellation by Brad Clark:  If the show is cancelled by Brad Clark, any money already paid is fully refundable to the purchaser.  Brad Clark shall not be responsible to any additional damages or losses suffered by the purchaser, for performance failure in such failure is to do any of the following:

a.       Physical disability or sickness of Brad Clark and / or death of immediate family member(s).

b.      Acts of regulations of Public Authorities, labor difficulties and / or strike, interruption or any delay of transportation service for any reason.

c.       Any other cause beyond control of Brad Clark.

 NOTE:  If a rider is attached to this contract it becomes a binding part of this agreement and must be signed and completed along with this contract.  Please return a signed copy of this contract and rider along with performance fee deposit within 10 days of receipt.

A map to the performance location and lodging location must be supplied at least 10 days in advance of show.

Video or audio taping of any kind during the performance is strictly PROHIBITED, without prior written consent of Brad Clark.

THIS CONTRACT AND ANY ATTACHED RIDER CONSTITUTE THE ENTIRE AGREEMENT BETWEEN THE PARTIES SIGHNED HERETO:
